INSTRUCTIONS to Become a Member

Outlined below is the process to become an official Assured Performance Co-op Member:

1. EDIT BUSINESS PROFILE - Complete the Full Business Profile by answering the questions to your best ability. The first part of the information is operating as a collective buying group. THIS INFORMATION IS FOR INTERNAL USE ONLY. ALL OF THE INFORMATION YOU SUPPLY IS STRICTLY CONFIDENTIAL AND WILL NOT BE SHARED WITH ANY OTHER PARTIES IN WHOLE OR PART.
   
   If you do not have specific information as requested, skip it and answer all of those you are able to. Later, you can update and complete any information you may have missed.
2. CO-OP AGREEMENT - The membership agreement is attached for you to review and sign. Once you have successfully completed the first step (editing your business profile), please sign the agreement acknowledging you desire to participate. By signing the agreement, you are officially becoming a participant in a legal co-op buying group program (Assured Performance Cooperative). This is a non-binding agreement. It is necessary to ensure that you have officially elected to participate. Fax this document to: 866-283-4512.
3. FINANCIAL REWARDS - Once you have filled out the business information and signed the agreement, you will be entitled to financial rewards from our participating Preferred Vendors.
   
   You will receive rebates and special discounts as part of the co-op. Depending upon the vendor, you may need to sign additional documents called �Patronage Agreements.� Your Account Coordinator will contact you where and when necessary.
   
   Once the agreements are received and processed, you will begin to gain rebates on the related products and services.
   
   NOTE: All participation is voluntary! Most agreements are in addition to any discounts and agreements you currently have and do not require changing manufactures, vendors, distributors, or jobbers. Please disregard any agreements that you do not want to utilize.
